O exército de jalecos brancos mostra como superar o coronavírus e futuras pandemias

Enquanto socialistas pagam um alto preço político por expressar uma opinião positiva sobre Cuba, a atual pandemia comprovou ao mundo inteiro o lado heroico do sistema de saúde cubano.

Vi esse heroísmo em primeira mão quando trabalhei com médicos cubanos em aldeias remotas e pobres na África. Era década de 1970 e eu era uma jovem que trabalhava como nutricionista na Organização das Nações Unidas para a Alimentação e a Agricultura (FAO). Meus colegas eram pessoas boas que estavam ajudando a alimentar crianças necessitadas. Eles também recebiam altos salários e viviam um estilo de vida riquíssimo, incompatível com o que teriam em seus países. Os cubanos eram diferentes. Eles viviam de maneira simples, trabalhavam sob as condições mais adversas e não ganhavam quase nada pelos serviços prestados. Sua motivação era puramente ajudar as pessoas que precisavam.

Eles chamavam isso de “internacionalismo” e disseram que era um dever revolucionário pagar a dívida que tinham com a sociedade. Eles citavam Che Guevara: “A vida de um único ser humano vale um milhão de vezes mais do que todas as propriedades do homem mais rico do mundo.”

Fiquei inspirada e acabei me mudando para Cuba. Quatro anos, um casamento e um bebê depois, fui acusada pelo governo cubano de escrever artigos críticos contra a revolução e acabei sendo deportada. Certamente, vi e experimentei aspectos do sistema cubano dos quais não gostava, mas nunca perdi minha admiração pelo sistema de saúde pública do país e pelo compromisso com a solidariedade internacional.

É realmente inspirador que esta ilha pequena e pobre tenha indicadores básicos de saúde iguais ou melhores aos dos países mais ricos do mundo. Isso é ainda mais notável depois de enfrentar 60 anos de bloqueios e sanções brutais dos Estados Unidos. A taxa de mortalidade infantil em Cuba de 4 por 1.000 nascidos é menor do que a dos Estados Unidos, de acordo com a CIA. Há pouca comida nas prateleiras dos mercados e faltam produtos nas farmácias, mas, como dizem os cubanos, “vivemos como pessoas pobres, mas morremos como pessoas ricas”. Isso porque a expectativa de vida de 79 anos é a mesma dos Estados Unidos, apesar de Cuba gastar menos de US$ 800 por pessoa por ano em assistência médica, em comparação com os US$ 11.000 gastos nos Estados Unidos.

Como a maior parte do mundo, Cuba agora está lutando contra o coronavírus. Em 20 de abril, havia 1.137 casos confirmados, com 38 óbitos. Mas o sistema de saúde gratuito e universal do país, incluindo um quadro robusto de profissionais de saúde, coloca a ilha em melhor posição para lidar com essa crise do que a maioria dos países. Com seu intenso foco na formação de profissionais da saúde, Cuba tem a maior densidade de médicos do mundo. A proporção entre profissionais de medicina e pacientes é aproximadamente três vezes maior do que nos Estados Unidos.

Cuba não apenas forma seus próprios médicos, como também forma profissionais de todo o mundo. A ilha de apenas 11 milhões de pessoas abriga a maior escola internacional de medicina do mundo, a Escuela Latinoamericana de Medicina (ELAM). Desde a fundação, em 1999, a escola formou mais de 35.000 jovens de 138 países, incluindo dos Estados Unidos. E aqui está o pulo do gato: o ensino é gratuito.

O Pastors for Peace, um grupo que seleciona estudantes estadunidenses provenientes das comunidades “mais humildes e carentes”, diz que as bolsas incluem aulas completas, alojamento em dormitórios, três refeições por dia, livros didáticos, uniformes escolares e uma pequena bolsa mensal. Para Cuba, a assistência médica tem sido uma marca da revolução: ajudando as vítimas do terremoto no Chile em 1963, nicaraguenses e hondurenhos devastados pelo furacão Mitch em 1998, vítimas do tsunami na Indonésia em 2004, haitianos após o desastroso terremoto de 2010 e o subsequente surto de cólera. Também foram enviadas equipes para a Libéria, Guiné e Serra Leoa para combater o Ebola em 2014.

Gradualmente, esse “exército de jalecos brancos”, como Fidel Castro os chamou, não apenas respondeu a emergências no exterior, mas passou a servir como médicos de família em comunidades pobres em todo o mundo. Os países mais pobres pagam apenas as despesas das equipes médicas ou buscam apoio internacional para compensar Cuba. Os países mais ricos pagam mais.

O governo Trump representa um desafio significativo aos programas sociais cubanos. Quando Trump chegou ao poder, seu governo lançou um ataque implacável à economia cubana: colocando novas restrições às viagens dos EUA à ilha, reduzindo a quantidade de remessas que os estadunidenses de ascendência cubana poderiam enviar para casa, interferindo no transporte de petróleo venezuelano para a ilha e tentando sabotar as colaborações médicas de Cuba.

Os fanáticos militantes anticuba no governo Trump incitam médicos cubanos que trabalham no exterior a desistirem do trabalho, pagam jornalistas para escrever histórias negativas, aplicam sanções contra os cubanos responsáveis pelo programa e armam países para expulsarem médicos cubanos.

O ponto crucial do ataque foi pintar o programa como uma forma de escravidão moderna, porque os médicos recebem apenas cerca de um quarto do dinheiro que os países pagam por seus serviços. Mas os profissionais de saúde cubanos são voluntários nessas tarefas: eles querem a experiência, ganham muito mais do que ganhariam em casa e sabem que o restante do dinheiro é destinado ao apoio ao sistema nacional de saúde de Cuba.

O governo Trump conseguiu convencer os governos de direita que chegaram ao poder na Bolívia, Brasil e Equador desde 2018 a mandar para casa cerca de 9.000 cubanos. Em uma reviravolta trágica, esses mesmos países agora estão sobrecarregados com o coronavírus e lamentam a perda de profissionais experientes.

À medida que avançamos nessa crise, vemos a tragédia da superpotência rica com seu sistema de saúde privatizado e disfuncional falhando com o povo, enquanto o vizinho pobre, sob constante ataque da superpotência, ajuda o mundo inteiro. Vemos Trump desesperado para evitar os desastres catastróficos dessa pandemia (inclusive retirando apoio financeiro destinado à OMS, em um movimento que um prestigiado editor de medicina considerou “uma terrível traição à solidariedade global”), enquanto o exército de jalecos brancos de Cuba se tornou a personificação da solidariedade global.

Se você tem ambições políticas, pode pensar duas vezes antes de dizer algo de bom sobre o sistema de saúde de Cuba.
